Once the lactation service is arranged on a certain date, it can only be moved, without monetary penalty, if notification is given more than 24 hours in advance. Small time adjustments are understandable, but if the requestor cancels the appointment within 24 hours of the appointment, $50.00 will be charged to the requestor. 

At the appointment, a HIPPA handout will be given. Also, a Release of Liability and Consent Form for Lactation Visits must be signed by the parent, before or at the start, of the appointment. A copy of the lactation chart can be copied to the parent for your plan of care. This plan of care can also be given to your baby's physician. This lactation chart can also be given to the parent for assistance with possible reimbursement by insurance but no guarantee is given on your reimbursement.
